Continuum of Care is a system that guides and tracks patients over time, spanning all levels of intensity of care. Avatar Home Health Agency offers home health, hospice, and personal care services which help our patients manage symptoms at home and detect problems before they start.
Continuum of Care refers to bundled care options that follow an individual through time, adapting to their changing needs. There must be a continuing health need that is being monitored, however it covers all levels from Home Health to Intensive Hospice Care. Continuum of Care is important to families and patients alike, and it leads to an improvement of satisfaction, reduces stress, and improves health. It also is beneficial to the patient’s well-being and quality of life. By choosing one company that knows the patients history, you are given the worry-free feeling of not having to look for another agency, and having the same caregiver throughout the duration of services. There is no longer the issue of moving from facility to facility or having to deal with multiple companies. If the patients health declines, we will be here in all aspects and phases.

RN Supervised
Direction from a trained professional allows you to draw from their wealth of knowledge when developing your plan. Our RN case manager, working in conjunction with you and your loved ones, will establish an Individualized Service Plan. Your Caregiver will keep detailed notes of services provided thereby keeping our supervisor, you, and your loved ones, up-to-date.
Open Communication and Status Reporting to Families
After your first scheduled appointment with your Caregiver we will contact you to make sure you are completely satisfied with the services provided and the Caregiver providing them. From then on we will make regular calls to ensure that you and your loved ones are pleased with the continued services. If ever a concern arises we are there to answer any questions you or your loved ones may have.
High Hiring Standards
We understand that having confidence in your living situation is vital and that is why we only hire experienced caregivers with a background in excellence. Therefore our Caregivers must go through two interviews and have a minimum of two years experience in the industry confirmed through rigorously checked references. They must also have a clean background and their credentials verified.

HOSPICE TEAM
Our Avatar Hospice team under the direction of the patient’s physician and/or Avatar’s medical director develops a personalized plan of physical, emotional, and spiritual care. The plan is flexible and designed to be adjusted as the needs of the patient change.
Avatar Hospice Team Includes:
- Nurses to visit patient as often as needed. Caring for the patient and teaching the caregiver to give the patient the best possible care.
- An aide to provide personal care including bathing, changing linens, helping with light housework and more.
- A medical social worker to help the patient and family cope with the stresses of a serious illness. The social worker also assists with financial and insurance issues and connects the patient and family to available community resources.
- A qualified volunteer who helps with a wide range of activities of daily living.
- A pastoral counselor to advise and assist with spiritual issues.
