The patient is in charge of the comprehensive care plan, which is implemented by our
hospice team. This team consists of experienced nurses, a chaplain, the patient's
physician, a social worker, a medical director, and a Hospice Aide

Our interdisciplinary team of healthcare professionals, including physicians, nurses, and
palliative care specialists, collaborates to develop personalized care plans focused on
